Imagine a single day when all the filtration systems supporting our lives failed! The importance of filters can be well anticipated even without such adverse events coming true. But how well are we aware of a natural filtration system which purifies the most important fluid in our body – our blood.
The kidneys a pair of bean shaped organs are responsible for filtering the blood and excreting the waste materials in the form of urine. Also the kidneys play an important role in the formation of blood cells and controlling the volume of fluid like lymph in the body. Kidneys also regulate blood pressure and maintain the calcium levels of the bones.
Various underlying disease conditions cause a failure of the functioning unit of the kidneys i.e. the Nephron causing it to lose its filtering ability. The patient is then advised dialysis and kidney transplant is the required corrective action in such conditions.
Kidney Transplant – The prerequisites:
Kidney Donor:
- Living Donor: The donor may be a living person who is usually a relative of the patient.
- Deceased Donor: Persons who are brain dead can be donors if their families permit.
- Blood Group Matching: Donors and recipients must have a matching blood group. The donor with Blood group O can donate kidneys to patients with blood group A, B and AB. However, blood group O can receive kidney only from donor with blood group O.
- Tissue Matching: Blood test for Human Leukocyte Antigen (HLA). Antigen are individualising markers found in most of the blood cells.
- Cross match: Human body produces antibodies in response to any infection or kidney transplant. If these are produced, the recipient’s body may destroy the kidney. Hence needs to be matched prior.
Kidney Transplant – The procedure:
The procedure lasting for 2 to 4 hours is a Heterotopic transplant wherein the transplanted kidney is placed at a different location. This procedure is carried out under general anaesthesia. The original kidney is retained unless there are serious conditions associated with it like excessive enlargement, recurrent infections and uncontrolled hypertension. The Artery supplying blood to the kidney and the vein carrying the blood away is connected to the artery and vein already present in the recipient’s pelvis. The tubes carrying urine are connected from the kidney to the bladder. Period of hospitalization is 3 to 7 days.
Low cost transplantation is carried out with a high success rate in India.
Kidney Transplant – The likely complications and solutions:
Like any other surgery, there can be complications though rare, due to:
- Bleeding, infection, or wound healing problems
- Difficulty in the circulation of blood to the kidney or problem with the flow of urine from the kidney
Both of which can be corrected by another surgery.
Rejection: the advancements in the post transplant and immunosuppressive medicines. Have reduced the chances of rejection but require a regular follow up from patient.
